cCAMP Affiliate Agreement Form

With respect to the recent incident involving recipient of cCAMP, World Ebony Network (WEN), a cultural Competence 501c3 Educational and Charitable organization, is pleased to establish a working relationship with your family.

WEN’s Cultural Competence Advocacy & Mediation Program-Prevention & Diversion System (CCAMP-PDS) is a restorative justice program that achieves WEN’s vision to advance the socio-economic community members and to ameliorate solutions within the community. As a networking organization, we work in collaboration with affiliates to achieve this noble goal. Some of our activities include but are not limited to ethnically driven research, discovery, and court intervention with ethnically appropriate restorative arbitration that includes counseling, mentoring, etc.). Our partnerships are not limited to courts, elderly people’s homes, health professionals, embassies, schools, caregivers, legal groups, cultural associations, institutions, and professionals. Some of our partners include Nigerian American Lawyers Association (NALA), Embassy of the Federal Republic of Nigeria, Washington DC, Council Members, etc.

It is on record that WEN has appeared and intervened in cases before judges at the following courts:

  • Superior Court/District Court, 500 Indiana Ave NW, Washington, DC 20001
  • Circuit Court for Anne Arundel County 8 Church Cir, Annapolis, MD 21401
  • United States District Court District of Columbia, Washington DC, 20001

Because meeting and managing stakeholder’ expectations, particularly those of legal professionals (attorney, judges, etc.) for these cases require being mindful of the state declaration of rights as well as other laws, WEN will appreciate proper activity coordination with respect to this ongoing case to get the maximum results. Our experience shows that some attorneys are non-corporative and shut down when they perceive too many voices communicating with them on a case while others do not.

To close or minimize the constraints that are likely to impact the successful outcome of this case, we request you agree that WEN takes the lead role (see below) of coordinating stakeholder meetings and keeping all informed in a timely manner, which includes written status reports. Hence, your timely response is highly appreciated.

For your information and records, this letter is being copied to all our partners associated with this case. To ensure the integrity of WEN’s activities, information and disclosures are treated as a matter of principle and legal requirements on the STRICTEST CONFIDENTIALITY. As such, you are required to sign the attached confidentiality, non-disclosure documents, to allow for free flow of information amongst partners involved in this case.

Explanation of Lead Role

While WEN takes the lead role, it is not insinuating that other stakeholders are less inferior or are subordinates to WEN. WEN leadership role is simply a management one to coordinate and manage the communication flow among stakeholders required to bring out a more effective result in any case. While lead role activities are executed on a case-by-case basis, please provide the following:

  1. Once you have signed our partnership agreement, please provide the information on:
    1. Attorney on this case – Whether you, the responsible relative of the Adult subject of this case, have retained legal counsel or one is assigned by the court.
    2. Detention, jail, prison, where the victim is held
    3. Other stakeholders – family, friends, etc., involved in this case.

  2. Where possible, we will convene individual or group meetings, preferably at our office. Bear in mind since WEN is offering this service to you and your family at no charge, we prefer to utilize the least expensive resource, which is our office. We do know that we incur the expenses of jail visitations, court appearances, lawyer visits expenses, etc.
  3. Character reference letters – With provided contact information of family and friends, WEN reaches out and requests character reference letters that the judge gives weight to in his sentencing.

  4. Scheduling visitations to conduct enlightenment sessions with inmates.

Please review this confidentiality agreement and statement of understanding and affirm you agree with it.


Partnership Confidentiality Agreement

I agree that all information shared with me is strictly confidential. With respect to this matter, I acknowledge that from time to time, I may need to disclose elements of the communications to WEN as part of my participation, progress, progress/status reporting, and tracking process.


I understand there are a few situations where WEN is required by law to report to authorities:

  • If any information is disclosed about child abuse or neglect
  • Substance, behavioral, or any kind of abuse to self
  • If any information is disclosed about the abuse, neglect, or financial exploitation of a senior citizen who is unable to seek assistance for himself or herself
  • If suicide is threatened
  • If bodily harm or death to another person is threatened
  • When a crime has been committed
  • Undisclosed Communicable disease

I understand that any breach of this confidential communication will result in immediate termination of this relationship without further notice.
